Telehealth informed consent

What you are agreeing to

This is the informed consent that Venezuelan telehealth regulation (article 12) requires before a remote medical consultation: no one may treat you over video without your written authorisation, and that authorisation is stored alongside the appointment.

Read it in full. By accepting it you declare that you understood it and that you consent to being seen by video consultation in the appointment you are booking.

What a video consultation is

A video consultation is a real medical consultation — with a verified, identified professional whose licence is active — that happens over a video call instead of in a consulting room. The professional can listen to you, see you, review what you share, advise you, order tests and, where appropriate, issue a prescription or a report.

The call runs on Google Meet, through the link shown in your appointment detail. Altiora Tech, C.A. does not record the call and does not store its audio or video.

What it is NOT

  • It is not an emergency service. If you have chest pain, difficulty breathing, heavy bleeding, loss of consciousness, signs of a stroke or any situation that puts your life at risk, do not use the video consultation: go to the nearest emergency service or call your local emergency number.
  • It does not always replace an in-person consultation. Some diagnoses require a physical examination, auscultation, palpation or tests that cannot be done remotely.
  • It is not an anonymous second opinion. A video consultation creates a doctor-patient relationship with the same duties and the same rights as an in-person one.

Limitations and risks you accept

Being seen remotely means accepting, together with the professional, limitations that would not exist in person:

  • The professional cannot examine you physically. The assessment rests on what you describe, what can be observed through the camera and the studies you share.
  • Connection quality may degrade the image or the sound, or interrupt the consultation. If the interruption prevents continuing, the professional decides whether to resume, reschedule or refer you to an in-person visit.
  • The professional may conclude, at any point, that your case cannot be handled remotely and ask you to attend in person. That is a clinical decision and it is binding, for your safety.
  • No transmission over the internet is infallible. Amedi encrypts your data in transit and limits who can see it, but a residual risk of interception exists in any electronic communication.

What you need on your side

  • A device with a camera, a microphone and a stable connection.
  • A place where you can talk about your health privately. Who else is in the room with you is your decision and your responsibility.
  • Your studies, current treatments and allergies at hand.

What gets recorded

This video consultation leaves a record in your clinical history inside Amedi: the date, the professional, the reason, the clinical notes, the instructions and any attached documents. Of your consent we store the version of the text you read, the language, the date and time, and the IP address and browser you accepted it from. The video call itself is not recorded.

Your health data is handled under the Privacy policy, and only the treating professional and their authorised team can access your clinical history.

If the person being seen is a minor

When the consultation is for a minor, consent is given by their parent or legal guardian, who must identify themselves by full name when accepting and be present during the call. The professional may suspend the consultation if they find the minor is alone.

Your rights

  • You may withdraw this consent at any time, before or during the consultation, without giving reasons and without affecting your right to be seen another way.
  • You may ask for an in-person consultation instead of the virtual one.
  • You may request a copy of what was recorded in your clinical history by writing to soporte@amedisalud.com.
  • You may report any improper conduct at /reportar.

Declaration

By pressing «I accept the consent» you declare that:

  1. You read this document in full and understood what the video consultation involves, its limitations and its risks.
  2. You had the opportunity to ask questions and to choose an in-person consultation instead.
  3. You freely consent to being seen by video consultation in this appointment.
  4. The information you provide is true and, if you are acting as the legal guardian of a minor, you have the authority to consent on their behalf.
